Healthy Family Support Practitioner – Full-Time Vacancy Working with children and or Young People 5- 9 years old and their families -in The Brentwood Area

 

 

We are seeking a passionate, dynamic and experienced 5-19 yrs practitioner to join our Healthy Family Team.
Our teams are fully integrated consisting of Health Visitors, School Nurses, Support Practitioners, Family Hub Practitioners and Community Connectors.

0-19yrs Healthy Family Support Practitioner Role

  • To work with childrenyoung people and their families, on a one to one or group basis, using a variety of interventions, to identify needs and to achieve positive outcomes.
  • To work in partnership with parents/carers to empower them and to enable them to achieve better outcomes for their children.
  •  To work co-operatively with children's serviceshealth agencies and other community based services, to provide the most effective service for children young people and families.
  •  Undertake developmental and health reviews of children and young people at key stages of their life

 

0-5yrs on occasions

  • Working in an integrated team to support the delivery of the Healthy Child Programme
  • Supporting the delivery of Maternal Early Childhood Sustained Home visiting programme (MECSH)
  • Promoting first relationships and attachment
  • Promotion of the Baby Friendly Initiative (BFI)
  • Providing universal and targeted support to families through delivery of group sessions
  • Working in partnership with individuals and their families to achieve positive outcomes
  • Deliver evidenced based parenting support to all families
  • Providing evidence-based interventions with families to support their children to become School Ready and meet age appropriate levels of development
  • Organising and undertaking Integrated two-year reviews in partnership with the family and the child's Early years setting
  • To participate in and deliver ECFWS virtual sessions to ensure all families are reached and have meaningful contact and achievable outcomes
  • To make contact with families where parental conflict has been identified as a ‘risk' factor, providing brief intervention support and advice
  • Participating and leading on health promotion activities in the family hubs and delivery sites or at external venues and events

5 – 19 (25 SEND)

  • To make contact with families where parental conflict has been identified as a ‘risk' factor, providing brief intervention support and advice ·
  • Provide Coaching and Skills training to young people and their parents to support the development of emotional literacy and overall wellbeing
  • To Deliver Educational sessions to Parents, Children and Young People adapting style and content to meet the audience.  ·
  • To work in partnership with other specialist providers in providing dedicated support to families of children aged 6-19 with a view to minimising harm and building the resilience of children and young people
  • To work in an empowering way to develop self-confidence, self esteem and emotional wellbeing within families · Support the development of Health Family hubs and Delivery sites as sanctuaries for families fleeing domestic abuse

Essential requirements for the role

Qualification mandatory for practice –A  minimum NVQ Level 3  

have experience of working with children within School, Home and community settings

Confident when delivering advice and support to Parents and Carers

Have a good understanding of accurate recording            

 Have a sound knowledge of GDPR

 have experience and empathy when building relationships with vulnerable children and adults.

As per NHS guidance this role may require successful applicants to evidence immunisations or immunity for MMR and Chicken Pox. This is due to the clinical focus of some tasks and the requirement to work with vulnerable families, young people and children.

The roles within this service may also require the successful applicant to wear PPE including the wearing of a protective mask and to follow social distancing rules as highlighted in NHS guidelines.  Successful applicants will be required to remain compliant with any future mandatory requirements introduced by the Government/NHS.

The Essex Child and Family Wellbeing Services (ECFWS) brings together a range of existing services, having been commissioned by Essex County Council and the Hertfordshire and West Essex Integrated Care Board.

The service, which is provided by HCRG Care Group in partnership with Barnardo's, ensures that families continue to have easy access to local serviceswith staff able to reach children and families in the community; whether in one of our Family Hubs or Delivery Sites, an Early Years Setting or School, or in a family's home.
